Report a Major Unusual Incident

Health and safety welfare concern?

​The MUI (Major Unusual Incident) Hotline staff address concerns for the safety, health, and welfare of people with a developmental disability. We take calls from many interested parties, including family, friends, coworkers, teachers, and other professionals. We’re on call 24 hours a day, 7 days a week. We’ll make sure the incident is investigated and that any follow-up needs are addressed.

​Who reports MUIs?

  • People with developmental disabilities regardless of service reception
  • Providers of service both in the home and in the community
  • Family members and caregivers
  • All county board staff
  • General public

​To whom are MUIs reported?

  • ​Reports can be made to any County Board staff
  • In person
  • County Board phone number – (937) 592-0015 (option #2)
  • Reports can be made directly to the Ohio Department of Developmental disabilities (DODD) if Local County Board is unavailable or anonymity is desired (800) 617-6733

​Who investigates an MUI?

  • MUI’s are investigated by WestCON COG via contract with the Logan County Board of DD.

​What is the role of the Ohio Department of Developmental Disabilities?

The DODD provides oversight and technical assistance. ALL MUI reports are reviewed by the DODD MUI/Registry Unit to ensure immediate action, timely reporting, good investigations and necessary implementation of prevention plans.DODD also conducts investigations where it would be a conflict of interest for the Logan County Board of DD to complete the investigation/report.

​What is the role of the provider?

  • Ensure health and welfare by taking immediate action to meet the need of the person served.
  • Report all incidents or allegations of abuse, neglect, exploitation, misappropriation, suspicious or accidental deaths to Logan County Board of DD immediately (no later than 4 hours of being made aware)
  • Reported regardless of where and who was present
  • Report all incidents or allegations of known/unknown injuries, Law Enforcement involvement (charges, incarceration, and/or arrests), Medical emergency, missing individual, Peer to Peer acts, prohibited sexual relations, rights code violations, unapproved behavior support, and unscheduled hospitalizations by 3:00pm the next business day.
  • Reported only when person is with a provider.
  • Implement prevention plans to reduce future incidents.

​What is the role of the Logan County Board of DD?

  • Ensure health and welfare of by identifying and supporting provider in taking immediate actions to meet the need of the person served
  • Work with provider related to prevention planning.
  • File report with DODD
  • Work with families, providers, and people receiving services to understand the MUI investigation process
  • Receive reports from community members, families, people served, and providers related to health/welfare of those served

​Why are families members contacted for information?

  • The MUI system is set up to help protect all individuals in the Developmental Disabilities system. By looking at individual and group data, we can see trends that can be addressed leading to better protections. These trends and patterns may include:
    • lack of access to adequate health care;
    • lack of diagnosis;
    • lack of assessment of symptoms;
    • lack of identification of new problems for people with certain syndromes;
    • Reporting helps ODODD and Logan County Board of DD to get a better picture of what is happening and helps both Departments to make improvements in the system that benefits everyone.
