Discovery Center Spring Program
The Ohio Self Determination Association in partnership with some other advocacy organizations are interested in hearing from people with disabilities, family members and others about:
To hear from as many as possible, a simple, short survey has been created. It has fewer than 5 questions and asks that you share information about yourself so that we can know where you live in Ohio, your age and your role (person with a disability, family member, other). There is a place to give us your name and email address, but it is not required, unless you want us to stay in touch and have us share the results with you later.
We hope you will take the time to complete the survey! You will be asked to set up an account with a user name and a password.
